As a Director of Nursing, one of the most crucial skills your team needs is the ability to document effectively.


Nursing documentation isn’t just about writing notes—it’s about accurately telling the story of your patients' care.


But let’s be honest—learning how to document isn’t always easy.


I remember when I started my career as an LPN, I had no clue what to document or when to document. 


I’d often ask my colleagues to read my notes, secretly hoping I’d done it correctly. The funny thing is—we were all in the same boat!


Documentation wasn’t something we learned overnight; it came with practice, guidance, and a lot of trial and error.


As I grew in my career, I realized that the level and complexity of documentation evolved with my skillset.


What worked as an LPN didn’t always translate as I became an RN and eventually a leader.


That’s when I understood: Nursing documentation isn’t just a task—it’s a skill that grows with you.


Why I Created the "Introduction to Nursing Documentation" Course

Over the years, I’ve had countless encounters with nurses struggling with documentation.


Some situations were minor—maybe an omission or unclear note that didn’t lead to any harm.


But in other cases, falsified or incomplete documentation led to serious consequences for both patients and staff.


These experiences taught me a valuable lesson: Proper documentation is non-negotiable. 


That’s why I created a short course titled "Introduction to Nursing Documentation."


This course is designed to simplify nursing documentation so that every nurse, whether new or experienced, can feel confident.


This is just the beginning! I plan to release additional modules in the future to dive deeper into specialized areas of documentation.
